“Monument Health has a long history as a resource for families facing one of their most difficult moments,” said Hospice Director, Michelle Sieveke. “We have been making a difference in the Black Hills for generations.”

In 2007, the hospice program proudly partnered with the Rapid City community to open the only hospice house in western South Dakota. Although the facility is minutes away from Monument Health Rapid City Hospital, the views that can be enjoyed from each of the 12 private rooms are reminiscent of idyllic Black Hills scenery.

The Home+ Hospice team prepares an individualized care plan that may include pain & symptom relief, personal care, spiritual support and counseling for you and your family. For patients at home, family and loved ones provide most of the hands-on care. Hospice staff visit regularly and are always available to answer questions. Monument Health provides in-home Hospice care through the Black Hills area. You can receive hospice care in your home, a nursing home, assisted living facility and the Home+ Hospice House.

Hospice care extends to every aspect of a person’s life — including their family. “Families and caregivers create a special bond,” Michelle explained. That bond continues following the passing of a loved one, as family members are eligible for up to 12 months of grief support from Monument Health caregivers.
